Meshima (also known as Sang Hwang in Korean and 'Black Hoof Mushroom' in English) is a slow-growing, wood-inhabiting polypore that forms hard, hoof-shaped fruiting bodies on mulberry trees. It is arguably the most under-recognized medicinal mushroom in the West despite being one of the most intensively studied in South Korea, where it has been the focus of a major government-funded research initiative since the 1990s. Its polysaccharides have shown some of the most potent immunomodulatory activity of any studied mushroom species.
Meshima has been used in Traditional Korean Medicine (TKM) and Traditional Chinese Medicine for over 2,000 years. Its name 'Meshima' comes from Meshima Island in Japan, where it was traditionally harvested. In TCM, it was prescribed for abdominal pain, intestinal hemorrhage, and menstrual disorders. In Korea, Sang Hwang was historically reserved for treating the most severe immune-related conditions and was considered the most valuable medicinal mushroom â even more prized than Reishi.

Meshima has been the subject of over 100 published studies, with the bulk of research originating from South Korean institutions. The Korean government designated Meshima a 'National Treasure' for medicinal mushroom research in 1993, funding an extensive multi-decade research program. Studies have demonstrated that its polysaccharide fractions â particularly a protein-bound polysaccharide called PL â exhibit potent macrophage activation, NK cell stimulation, and T-cell modulation. A 2004 study in Life Sciences showed Meshima polysaccharides to have stronger immune-stimulating activity than extracts from Reishi, Maitake, and Turkey Tail in comparative analysis.
